1). John had born with the infection of Transient Hypogammaglobulinemia of Infancy. The infant has lower IgGlevel compared to others with the same age. In THI, the infant has immunoglobulin <400mg/dl and also decreased the level of IgA, IgM. The immunity level is immature in this patient and prone to get a frequent infection. Ear and sinus infection is most common in THI. The causes may fail to produce B-cells, Immature B-cells, and genetic variation in certain families.

2). John was well during the first 10 months of life. Because during pregnancy, the fetus receives maternal IgG through the placenta. At the time of delivery the baby IgG level is equivalent to maternal IgG level. After birth, the baby does not receive maternal IgG and fail to produce on its own. The baby may get antibodies via breastfeeding. Once the maternal IgG falls, the baby slowly develops the hypogammaglobulinemia.
