Millie has one daughter, Diana Olsen, who is 50, that lives nearby, and Millie's major support system. Her current medical problems include: hyperyension, glaucoma, osteoarthritis of the knee, stress incontinence, osteoporosis and hypercholesterolemia.

Millie is at the clinic for routine examination follow up. She is taking several anti-hypertensive medications, diuretics, and analgesics. During the monologue, Millie provides important details of how she views her current situation.

Medications include:
50mg metoprolol PO BID
2mg lisinopril PO QD
240mg diltiazem PO QD
20mg furosemide PO QD
25mg hydrocholothiazide (HCTZ) PO QD
400mg ibuprofen PO BID
5/325 hydrocodone/acetaminophen (Lortab) 1-2 tablets PO Q8 hrs PRN severe pain

Several weeks have passed since the clinic visit, and Millie is now in the hospital with the diagnosis of a urinary tract infection and dehydration. Her presentation is atypical and she is confused.

At the 3:00 PM shift change. Millie has been in her room on the med-surg unit for about 6 hours. She was in the ER overnight because there was no available beds. Due to her confusion, Millie did not take her medications properly in the days prior to admission and as a result, her blood pressure is very elevated. Millie's daughter, Dina is at the bedside and is quite concerned about thr confusion and elevated blood pressure.

The major goal for this teaching plan is to take precaution with such a elderly patients. Such patients need more care and support during their treatment. A small mistake can became fatal for them.

There are several teaching resources for nursing which includes

· The American Association of Colleges of Nursing (AACN)

· Practical learning during internship

· Several nursing Resources & Education Content available on internet.

Specific instruction regarding medications is the proper administration of hypertensive medication because it prevents glaucoma. The adverse reaction may include risk of cardiac arrest, hemorrhage, coma, severe depression etc. So, it is needed to take extra care in such cases of hypertension with glaucoma

Participation of family members are very important in mentioned case, because in this situation, the patient is elder and she needs proper medical treatment with emotional support. She needs to feel emotionally secured and make to believe that she can be well soon.

At first, the patient needs to communicate with her family members and friends. She needs to share her problems with her caregiver too. It is helpful for the doctor also to understand her medical condition which results in better treatment.
