Option B and C.

Alzheimer's is a progressive disease which destroys memory and functions of the body.

When we provide stimulating environment for the client they will be able preserve their basic skills and they can enjoy and remenrem the things that they did.

Limiting the choices helps them because choosing between several may cause frustration for them and they may not have decision making capacity in a large scale.

Even if we place signs to locate the bathroom they may forget what they are going for or they many not recognise the toilet so it should not be done but they should be assisted.

Confronting will not help them one bit because they are not doing it intentionally and it is unethical to act rid towards the person.
