Based on your assessment data and the chart above, identify as many problems as possible(at least 10) for example, Pain, Anxiety, Infection:

Neurologic - A&O x 4. Pupils equal, round, reactive to light and accommodation. Patient has some level of slurred speech. Pain 0/10.

Cardiovascular - Normal S1 S2 heart sounds. History of primary HTN, anemia, and ischemic stroke. Capillary refill <2 seconds. Heplock on right hand.

Respiratory - Lung sounds clear on auscultation bilaterally. Smoker for over 50 years.

Gastrointestinal - Hyperactive bowel sounds on all four quadrants. History of constipation, type 2 DM (glucose level: 169 mg/dL), and GERD. Patient currently on 1500 calorie diet. Continent of bowel.

Urinary - No known GI issues. Continent of bladder.

Musculoskeletal - Ataxic gait. Fall risk.

Integumentary - Skin dry and intact. Normal color for ethnicity.

Reproductive - No biological children. Client has long-term girlfriend of over 20 years and one stepson. No known reproductive issues.

Spiritual - Christian (Catholic) faith. Prayer is a large part of the clients life and he enjoys reading “ Our Daily Bread” devotional.

Psychosocial - Client was born and raised in Washington, DC. He has a long-term girlfriend of over 20 years. He is a retired baggage handler at a local airport. Client also has a history of depression.

Development - Ego Integrity vs. Despair. Client displays ego integrity, “I’ve had a good life. Makes no sense to complain, because what can I really do? I’m just grateful to wake up.”
