fatality rate = (number of death / number of total population affected with disease) x 100

Fatality rate = (6000 / 34000) x 100 = 17.64%

So the above fatality rate of 28% is wrong. The correct fatality rate is 17.64%.

In above case, mortality and incidence data and the fatality rate cannot be correct at the same time. If the mortality data contains death due to other diseases or not including deaths due to the specified disease can change the fatality rate. Also if the incidence rate was not accurate fatality rate can change.
