One patient came with complaints of frequent urination and headache for one year.Upon history collection and physical examination his blood pressure is elevated and his blood sugar/glucose levels are high. He is suffering from hypertension,diabetes mellitus and heart disease. He has undergone blood sugar test and CT scan test whose findings are normal.Hence he got discharge from hospital
