Answer #1

3. A licensed practical nurse should perform a wet- to- dry dressing change for a client who has a pressure ulcer

  • Administering injections, prescribed medications, enemas etc., clean and dress wounds and assist with otherbasic medical care tasks is a responsibility of a LPN.

1. Assess a client's postoperative abdominal incision. Observing patients under treatment to identify progress, side effects of medications etc is the responsibility of a LPN. She don't have to assess the condition.

2. Provide discharge teaching for a client who had a laproscopic cholecystectomy. Communication of health related topics is offcource a responsibility of a LPN. but she should communicate with the patients' family or friends to provide advice, comfort, and release insruction and not to give education to the patient regarding a major surgery.

4. Initiate a blood transfusion for a client who has a low hemoglobin. This is the responsibility of Registered nurse.
