Montelukast 10mg

Brand name: Singulair

Generic name: Montelukast

Dose:

  • Tablet: 10 mg
  • Tablet, Chewable: 4 mg , 5 mg
  • Oral granules: 4 mg/packet

Frequency: Per oral once daily

Drug classification: Leucotriene Receptor Antagonist.

Indication/Use:

  • Used to control symptoms of asthama and allergic rhinitis.
  • Used in exercise induced bronchospasm.

Action: Montelukast is rapidly absorbed following oral administration.It is active leucotriene receptor antagonist that inhibits the cysteinyl leucotriene receptor. This receptor are released from nasal mucose after allergen exposure during early as well as late phase of reactions. Montelukast is orally active which binds with high affinity to cysteinyl leucotriene receptor.

Adverse side effect:

  • Headche
  • Dizziness
  • Heart burn
  • Stomach pain
  • Difficulty in breathing and swallowing
  • Hoarseness
  • Itching
  • Pain and swelling in sinuses
  • Flu like symptoms

Nursing Intervention:

  • Advice patient to inform doctor in case of any side effect with regard to this medicine.
  • Keep this medicines in room temperature in tightly closed containers.
  • Discuss about the history of asthama or related problems with health professional before taking medicines
  • Monitor vitals regularly.
  • Swallow regular tablet whole with a glass of water.

Irbesartan 300 mg

Brand name: Avapro

Generic name: Irbesartan

Dose: Available in 75 mg, 150 mg and 300 mg depending on the disease condition of the patient.

Frequency: Per oral once daily

Drug classification: Angiotensin Receptor Blocker.

Indication/Use:

  • High Blood Pressure
  • Diabetic Nephropathy

Action: Angiotensine II is the primary vasoactive hormone of the renin angiotensin system.and is an priority factor in the pathophysiology of hypertension.Irbesartan blocks the vasoconstrictor effect of angiotensin II by binding to AT1.Blocking of AT1 receptor removes negative feedback of angiotensin II on renin secreation which increases plasma renin activity and this helping in cardiovascular regulation.

Adverse side effect:

  • Hyperkalemia
  • Dizziness
  • Heart burn
  • Stomach pain
  • Fatigue
  • Reduced hypertension when in sitting or standing position
  • Impotence
  • Impaired renal function

Nursing Intervention:

  • Avoid foods and drinks that trigger hypertension such as alchohol, coffee/cafiene, salt, smoking

Metoclopramide 5mg 1v

Brand name: Reglan

Generic name: Metoclopramide Hcl

Dose:

  • Recommended dose{1-18 yrs} is 0.1-0.15 mg/Kg body weight. Maximum dose within 24 hrs can be 0.5 mg/Kg body
  • Recommended dose{For Adults} is 30 mg or 0.5 mg /kg body weight.

Frequency: Intravenous route thrice daily

Drug classification: Prokinetic Agent

Indication/Use:

  • Relieves acute and recurrent diabetic gastric stasis.
  • Treating nausea and vomiting associated with cancer chemotheraphy
  • Prevention of postoperative nausea and vomiting
  • To facilitate small bowel intubation

Action: Metoclopramide binds to dopamine D2 receptor with nanomolar affinity. The antiemetic action of metoclopramide is due to its agonist activity at D2 receptor which triggers the chemoreceptor zones of the central nervous system. It increses the motility of stomach, duodenum, jejunum and relaxes the pyloric sphincture.

Adverse side effect:

  • Drowsiness
  • Dizziness
  • Headache
  • Insomnia
  • Fatigue
  • Flushing
  • Diarrhea

Nursing Intervention:

  • Advice patient to drink lots of fluid.
  • Monitor vitals regularly.
  • Regular blood test to be done to check the electrolyte balance in the body.

Oxycodone 5mg

Brand name: Roxicodone

Generic name: Oxycodone

Dose: Available in 5 mg, 15mg or 30 mg.

Frequency: Every 4-6 hrs as needed.

Drug classification: Narcotics

Indication/Use:

  • Relieves pain when administered initially(such as pain of cancer)

Action: It is a semi-synthetic opioid.Its effects can be seen 1 hr after the administration and last for almost 12 hr. Most of the drug is metabolized in the liver and rest of the ones are excreted through kidney.Oxycodone binds to receptors and decreases excitability.

Adverse side effect:

  • Circulatory depression
  • Cardiac arrest
  • Insomnia
  • Hypotension
  • Vomiting
  • Nausea
  • Constipation
  • Pruritis

Nursing Intervention:

  • Donot use this medicines if patient has severe symptoms of asthama
  • Advice patient to drink lots of fluid.
  • Monitor vitals regularly.
  • Advice patient to inform doctor in case of any side effect with regard to this medicine.
  • Keep this medicines in room temperature in tightly closed containers.
  • Always monitor patient regularly while taking this medicines to see any changes as this medicines can cause very much complication in some patient with a single dose.
