Answer #1

3. The ordered dose of Benadryl is 25mg IV q 6 hourly

Weight of the child is 50lbs

Available Benadryl is 12.5mg /ml

Recommended dose of Benadryl for a child weighing more than 12kg is 5mg/kg/day in four divided doses

a) weight of the child - 50× 0.45( 1lbs =0.45kg )

= 22.5kg

b) Recommended safe dose for child is 5mg× 22.5 kg= 112.5mg in four divided doses , so single dose will be 28.1 mg

c) Yes the order is safe

d) Using unitary method :-

12.5mg of Benadryl is present in 1ml

1mg of Benadryl will be present in 1/12.5 ml

Then 28 mg of Benadryl will be present in 1/12.5× 28 = 2.24ml

4. Weight of the child is 28lbs = 28× 0.45kg = 12.6kg

For children 11-20 kg the daily fluid requirement is 1000 mL + 50 mL/kg for every kg over 10

Maintainence fluid - 1000ml + 50ml × 2.6

= 1130ml

Flow rate of the infusion pump = total volume /total time

= 1130ml/24hr

= 47ml/hr

5. The ordered dose of Omnicef is 70mg BD PO

weight of the child is 22lbs

Recommended dose of Omnicef is 14mg/kg/day twice a day for 10days

Available dose of Omnicef is 125mg/5ml

a) weight of the child is 22lbs = 22×0.45kg ( 1lbs = 0.45kg )

= 9.9kg

b) Yes the order is safe

Recommended dose = 14mg× 9.9kg = 138.6mg = 138.6/2 = 69. 3mg single dose

c) Using unitary method :-

125mg of Omnicef is available in 5ml

1mg of Omnicef will be available in 5/125ml

Then 70mg of Omnicef will be available in 5/125×70ml

= 2.8ml single dose , 2.8ml×2 = 5.6ml for a day

6. The ordered dose of Cipro is 300mg q12hr

Weight of the child is 30.3kg

Available dose of Cipro is 250mg/5ml

Recommended dose of Cipro is 20-30mg/kg/day q 12hrly

a) weight of the child is 30.3kg

b) Dosage range for this child is 20× 30.3mg- 30×30.3mg

= 606mg-909mg

c) single dose is 606/2 = 303mg or 909/2 = 404mg

d) Using unitary method :-

250mg of Cipro is available in 5ml

1mg of Cipro will be available in 5/250ml

Then 300mg of Cipro will be available in 5/250× 300

= 6ml
