A 50 year old man notifies his physician that he has passed approximately 12” of a tapeworm with a bowel movement. He has not had any abdominal complaints up to this event. The physician tells him that he probably has a Taenia species tapeworm – either the pork or the beef tapeworm. The patient explains to the physician that on religious grounds he is extremely careful to avoid eating any pork or pork products. The physician tells the man to take the tapeworm and two stool specimens collected a day apart to the laboratory, and sends an order to the laboratory requesting identification of the tapeworm to genus and species. The laboratory easily identifies Taenia spp. eggs in both fecal specimens.

How would you proceed with an identification to the species of this tapeworm ?
What precautions should laboratory personnel take here ?
Why is speciation important here ?

You determine that the tapeworm is Taenia solium.

The physician prescribes the appropriate therapy for the patient and then contacts the laboratory about examining patient feces for the tapeworm scolex as a “test of cure”.

What would you advise in this situation, and why ?
What should now be emphasized to the patient by his physician ?

Answer #1

After identification of the species of tape worm it is essential to do speciation .Along with this the other diagnostic tests like coproantigen or immunoassay can exactly detect and conform it.

This is contagious if an individual comes in contact with it and doesn't follow standard precautions. The laboratory professionals should use the personal protective equipments like gloves ,mask and goggles if there is chance if splashes. After the test the sample has to be discarded as per the guideline so that it cannot cause infection.

The speciation is important to identify the exact species in which the Taenia belongs to because there are chances the eggs and the proglotidds taken from the sample could make a new species in varied environments

The laboratory technicians can be adviced  to check this as a screening test for neurocystocercosis because the larvae can infect the brain

The patient should be emphasized on the following

  • Avoiding undercooked pork or avoiding it completely
  • Not to eat any foods from the places which are unhygienic, unclean and served in open place
  • Strict hand washing to be done
  • Complete the antibiotic course
  • Report in case of abdominal pain ,constipation, fever, changes in mental status or any other side effects.
