Nursing care plan:

Fractured hip:

- assess patient fractured site for any external injury

- immobilize the area to avoid further injury

- do routine investigation like XRay, blood investigation etc..

- assist patient for their elimination routine..

- administer pain medication as per doctor advice

Second day post op for hip arthroplasty:

- patient may experience with severe pain associated with higher risk of post operative delirium..As per doctor Advice administer opiate medication and regional nerve blocks to reduce the pain..

- DVT should be prevented especially for elderly patients.. administer Anticoagulant therapy and intermittent pneumatic compression devices to decreasing risk of DVT and pulmonary embolism..

- delirium is associated with lower functional level it should be prevented..Assess patient fluid volume deficit, pain, elimination pattern, oxygenation, and medication..

- support patient for early mobilization.

- Assess the surgical site for drainage, redness and swelling..

- have good communication with patient.

- Encourage and teach patient with physical therapy about little position change and post operative exercise to relieve pressure..

- make sure patient wear compression stockings to reduce DVT..

Post op care after amputation:

- monitor patient pain level and administer medication according to that..

- monitor patient wound healing process..

- physical therapy with gentle exercise and massage, stretching exercises can control the pain and pressure..

- provide emotional support and counseling to avoid grief..

- Avoid edema using compression socks.

- removable ridged dressing should be applied for ease of application and wound inspection..

Inpatient with traction:

- make sure that girdle is proper size for Patient..

- inspect the skin area for any pressure.

- follow hygeine after elimination..

- provide back care every 4hourly

- turn the patient little with use of trapeze.

- maintain bed with flat level.

Patient with cast:

- make sure physiotherapy perform upper and lower body strengthing exercise to assess activity intolerance.

- encourage patient to sit with bedside chair.

- assess patient range of motion exercise in upper and lower extremities.

- administer pain medication.

- check the skin for any discoloration and swelling..

- assist patient for their daily routine..

- provide counseling and psychological support to avoid low self-esteem..
