A patient was admitted with a type 1 ST-segment elevation myocardial infarction (STEMI). Patient was found to have an occlusion of the second diagonal branch between the two previously placed stents. The patient was taken to surgery for a percutaneous transluminal coronary angioplasty (PTCA) of the occluded vessel, with placement of two non-drug eluting stents. Patient also has hypertension, coronary artery disease (CAD), and previous PTCA with placement of a stent, but no history of coronary artery bypass graft (CABG).

Final diagnoses and procedure: (1) Acute anterior wall MI due to CAD, (2) hypertension, (3) previous PTCA with stent, (4) angioplasty and stenting of diagonal branch.
