COMPARING AND CONTRASTING THEORIES
I would like to compare and contrast two famous nursing models, Orem's self-care deficit theory by Dorothea Orem and Roy's Adaptation model by Sister Callista Roy. As the name implies the central concept of Orem's theory is about self-care whereas that of Roy's model is adaptation. According to Orem, self-care is the performance or practice of activities that individuals initiate and perform on their own behalf to maintain life, health and well-being. As per Sister Callista Roy, the goal of nursing is to help the person adapt to change in physiological needs, self-concepts, role function and interdependent relations during health and illness
Both theories look different on a superficial analysis, but at some point, i feel, these theories are integrated with each other. First and foremost example is care itself. it is the central and supreme thought without which nursing profession cannot go forward. Both theories give importance to the nursing practice, thus promote better client care, increase the nursing profession standard, and help to communicate with co-workers, and give supervision to the researchers and education. Both theories share the same nursing process; assessment, identify the problems, plan goal, intervention, and evaluation. The process and approach remains the same though they used different words for nursing.
person
Orem's theory consider patient and family as the social units.she perceives individual person as a whole and stresses the knowledge level, cultural values and beliefs are some of the factors that influence self-care practices of person. At the same time, Roy's theory says human as an individual on the basis of systems theory. It brings equilibrium in the individual so as to achieve healthy existence.
health
Health and illness are very much important in a person's life and nurse plays a vital role in promoting health. Both theories emphasize health as a state of being structurally and functionally whole or sound.
environment
According to Sister Roy, environment is the changing one that stimulates the person to make adaptive responses. But Orem never used adaptation in model but focused on individual capacity for self-care.
Now, I would like to select one model which aligns best for clinical scenario. It is nothing but Orem's self-care deficit theory. Reasons are, mainly, it is simple and can apply in all settings. It focuses patient and nursing care practices which involves family and community. Another reason is, this model is connected to practical science and thus leads to improvisation of nursing profession. Next is, Orem's concepts are internationally accepted and commonly used in nursing field. Being a nurse, I found that I am following Orem's conceptual model of self-care deficit everytime when i am taking care of a patient, unknowingly.
Nursing practices when supported with theoretical structure, it, indeed, serves as an organizing framework for a body of knowledge. To improve nursing care quality, everyone should utilize nursing theoretical models, so as to excell nursing care.
