In monitoring for various complications at XYZ Rehabilitation Hospital, health information manager Mary Moore discovers that in many instances patients with deep vein thrombosis or pressure sores are being admitted to the hospital with those conditions. Because rehabilitation cannot proceed until these medical conditions are resolved, these patients may be discharged from rehabilitation to another type of care after a short stay and later readmitted to rehabilitation. Prevention of deep vein thrombosis-
1. Drink plenty of fluids and avoid coffee and alcohol.
2. Don't wear short,tight socks and try not to cross your legs a lot.
Prevention of pressure sore-
1. Don't lie in same position for longer time.
2. Position should be changed for 2 hour interval.
3.keep skin clean and dry.if necessary apply powder in summer.
4. Exercise daily.(range of motion exercise).
5. Put pillows between parts of your body that place against each other.

Mary should tell the patient that after treating the deep vein thrombosis and pressure sore patient can come to the rehabilitation centre. in this way prolonged staying in the hospital may decrease .admission and discharge procedure also needs time and its a time consuming procedure and also needs to fill up so many form .so it will be helpful if they discharged the patient after rehabilitation care .so they don't need to readmission to the hospital.
