Rachel 50 years old, working in a super market. She has colosotmy due to colon cancer. her stoma healthy, and her post-operative recovery has been uncomplicated. Rachel state" my poo is coming out of my belly" she will not look at her stoma and does not want to take care of it. her husband is very supportive, but he is very uncomfortable when there is any care of exposure of a stoma. Rachel will be discharge tomorow. and need to able to care for her stoma.

You will be given a case study above with a client in need of health teaching for discharge. your teaching plan should include the assessent why the client and her husband need   teaching and also include the learning barrier

Health teaching :-

#. Health teaching or education is needed for this couple ( patient and his husband ) because the patient has a colostomy in place due to her colon cancer . It need to be taken care well to avoid complications such as infection , bleeding , blockage , hernia etc.

#. Learning barrier in the health teaching was the one and only unreadiness to take of it because it does not look good. It makes them feel bad when they see poo coming out of the belly .

#. Before the health teaching , i have to make them ready for the colostomy care . And for this i asked them to express their feelings and concerns regarding colostomy, then made them the patient accept that she has a colostomy and now it's a part of her and has to take care of it as a body part . Taught them about the complications it would cause if not taken care of .

Health teaching included the following :-

- how to change the colostomy bag and clean stoma

- assessment of atoma's health based on colour

- Avoid foods that cause flatus:

the bag will fill with air instead of stool, bad can explode

Drink 2 quarts of water daily:

keeps the stool softer and less formed, avoids blockages

- Teach about medications:

different medications will cause different interaction, people with transverse and descending stoma's are on stool softeners

Diuretics can cause dehydration which means the stool have less fluid

Certain medications can cause diarrhea

- Periodically dilate stoma:

dilate the stoma with a plastic syringe, dilate the stoma with water and then rinse

- Teach about odor control:

patient fear their ostomy will smell: as often as you can, empty your stoma at 1/2 way full, powder and spray deodorant can be administered to the bag

Evaluation :-
- After the health teaching , on evaluating the patient ,the main think i succeeded in is the patient was ready to take care of it and accepted it .
