Process of release of information:

General Authorization consent for Release of patient information is important..Authorization letter should be in written form to premiere medical group that must include

- name of the organization which is going to release the information

- patient name,age, address, date of birth with photocopy..

- name and photocopy of the person who is going to receive the information and their relationship (spouse, children)

- nature of information to be released including date of treatment received..

- date of consent and signature from patient and legal representative..

- photocopy of the original form ..

This Authorization will expire in 120days from the date of signature..it will take 30days for releasing the information..The premier medical group will consider the signature of the Patient to be valid for ROI if the patient is living, close relative of the person..transferring care to another facility, for any further treatment or investigation to be done..

Confidential information containing there record is not released to individual with out Authorization from the patient or legally authorised others like guardian..patient having rights to control their private health information..premier medical group and it's policy needs to be followed for any information about medical, social, personal and financial information of patients..premier medical group and in compliance with new york state public health laws and federal law concern with release of information..
