Nurse is planning care for a client who has dementia should include interventions based on the stage;

First stage: Work for ensuring the dignity and autonomy. Maintaining the; independence and enjoyment of the person suffering from dementia.

Second stage: Working to ensure safety as the person with dementia is at a higher risk of decline in function and insight, development of a changed and risky behavior.

Third stage: Working to ensure comfort, dignity, and quality of life, including the reassessing of medications and the different treatment approaches.
In general, the nursing intervention involves; assessing the conditions lead to an increase in the patient’s level of fall risk, recording a history of falls, mental status changes, sensory deficits, balance, medications, and the symptoms that are related to the diseases. Assessment of the risk of fall and making arrangements to make the environment safe.
