Answer :

Type 2 diabetes mellitus is a non insulin diabetics mellitus.

1. Assessment :

It have subjective data and objective data.

Subjective data : the client has been complaint about he is having excessive thirst and frequent urination and and he is loosing his weight , increased appetite.

Objective data :

Poly dipsia, poly urea, weight loss, poly phasia, fatigue, These are observed from client.

2. Diagnosis :

Hyperglycemia or increased blood sugars related to insulin resistance and relative lack of insulin as evidenced by the blood glucose levels are increased than the normal that is random blood glucose levels are 202mg per dl.

Activity intolerance related to lack of energy as evidenced by inability to perform daily activities by client.

Insomnia related to frequency of urination or poly urea during night times as evidenced by client verbalization.

3. Plan :

* to assess vital signs

* monitor skin turgor,

* assess weight of the client daily,

* blood glucose monitoring.

* to administer well balanced diet

* to administer anti diabetics drugs.

* to provide psychological support.

* to provide emotional and physical support.

4 .Implementation :

* checked the vital signs of client , some times due to infections there is a increased temperature.

* assessed skin turgor, poor skin turgor due to fluids loss.

* checked weight of the clients daily, there is a weight loss that is from 65 kgs to 63 kgs reduced with in two months.

* monitored blood glucose levels that is 202 mg per dl.

* provided well balanced diet that is low carbohydrate and high fiber diet.

* administed tab glimipramide per oral once a day 500 mg.

* provided psychological support and emotional and physical support to do activities.

5. Interventions :

There are what we will plan goals those are only interventions.

Goals are to maintain normal blood glucose levels.

To maintain normal body weight.

To maintain normal daily activities by client.

6. Evaluation :

* maintained normal blood glucose levels by using medications, that is 140 mg per dl post pondrial.

* the client body weight is maintaining.

* the client is performing daily activities.
