Q. No. 2. Answer :

From the clinical findings ABG ( arterial blood gas values) we can correlate with M. K'S chronic bronchitis.

Because PaCo2 is high that is 52 mm of hg, and PaO2 levels are low that is 48 mm of hg.

Normally PaO2 levels should be high in ABG.

If low PaO2 indicates patient is having breathing difficulty.

Treatment recommendations :

* drink plenty of fluids.

* take plenty of rest.

* to relieve pain use the Ibuprofen and aspirin.

* Nonsteroidal anti inflammatory drugs can be use.

" congestive heart failure " we can suspect, because distended neck veins, excessive peripheral edema and excessive urination at night times seen in these condition only.

So he is suffering from " congestive heart failure ".

Pathogenesis :

Renin angiotensin aldosteron pathway stimultates,

* angiotensin l converts into angiotensin ll.

* peripheral vasoconstriction may increases,

* increases over load,

* and decreases cardiac output,

* and it leads to heart failure.

Here the patient is having 158 / 98 mm of hg. So he is suffering from stage 2 hypertension.

Rationale for current medications for hypertension :

Lotensen : it is ACE inhibitor it converts the angiotensin l to vasoconstrictor substance angiotensin ll, and also stimulates the aldosteron secretions by the adrenal cortex.

So that decreases the blood vessels tightening and increases the blood and oxygen supply to the heart by that blood pressure may decreases.

Lasix : it is diuretic, it blocks the absorption of sodium, chloride and water from the filtered fluid from the kidney tubules and increases the excretion of urine to the outside.

So that excess blood pressure may decreases.

Glycosylated Hemoglobin for this patient is " 7.3 % ".

* the client is having high, normal levels should be below the 6%.

* above 6 % indicates patient having diabetemellitus.
