As soon as the defibrillator arrives deliver an unsynchronized shock of 2J/kg.if rythm check after 2hours of CPR reveals Ventricular tachycardia give 4J/kg, then resume compression.

If it is a pulseless Arrest or VT

Defibrillate at 2J/kg to a max 360J or equivalent biphasic energy. Epinephrine 0.01mg/kg IVP/IO 3-5min

Defibrillate at 4J/kg and then defibrillate within 30-60seconds after each medication throughout arrest.

Give 1shock of 4J/kg if using a manual defibrillator. If using an AED, in a child under 8years of age use paediatric attenuation, whenever possible use the adult shock energy(150-200J Biphasic ,360 monophasic)
