A. Al, a reformed alcoholic, is 42 years old with failing kidneys. He has been having hemodialysis 3 days/week for 3 years. He has had to deal with 5 failed accesses; is about to lose his recently revised access. Without kidney transplantation, he will die within the year. He is a father of 3 children, unemployed, and on welfare (Medi-Cal).

B. Sam, a well-known and respected Neurosurgeon, is a 50-year-old with failing kidneys, as well. Unlike Al, Sam currently drinks in excess (some might label him as an alcoholic). Sam is single, gainfully employed and is loved by his colleagues and friends.

1.Some of the reasons for Sam to get a kidney transplant are

  • His ability or his fitness and the positive outcomes of a transplant if undergone .
  • The rate of acceptance should be high with minimal or no rejection as a factor
  • In simple the survival rate can be higher and this procedure can save his life
  • Financially secured at the last

The reason to reject Al from a kidney transplant are

  • Poor outcome or very high risk for rejection
  • The survival rate is very low
  • Extreme weakness or unfit to undergo a surgery which can endanger his life immediately
  • Financial instability

2.Some of the debatable points are

  • The unethical nature to not take a patient for kidney transplant who is about to survive only a year without surgery.This can be because of the risk for failure of surgery and can make the patient more vulnerable and have a critical end of life time .
  • Al, can be considered in accordance to his age to be selected for transplant on ethical grounds. That this person is in his productive state of development and provided a chance to survive with transplant.
  • The loss which will incurred to Al's family after his passing out .The three childrens has to be grown up in an environment without father's presence which can be a emotional trauma to overcome for them
  • Sam ,selected for transplant is also a better option because if he recovers he can be beneficial to the society with adequate deaddiction programs.
  • The financial stability and security of Sam cannot be a reason for his selection because life is the equal rights of everyone universally.
