Weaning

Weaning from mechanical ventilation is a process in which ventilatory support is reduced which returns spontaneous breathing of patients, it gradually decreases dependence on mechanical ventilation after that patient extubated.

Process of Weaning

First of all, we have to see the underlying condition of the patient is improved or not( here the underlying condition is bacterial pneumonia, we can identify the improvement of the condition by seeing chest x-ray and WBC count ).

Then we have to see the respiratory parameters in arterial blood gas analysis if the partial pressure of oxygen is adequate, the patient is maintaining saturation with low FIO2 and partial pressure of carbon dioxide is in the limit. Then we can start the process of weaning.

First of all, we have to change the ventilator setting to a spontaneous breathing trial (SBT). Decrease FIO2 as much as possible according to patient saturation.

Reduce pressure support, if the patient responds well and initiating spontaneous breathing then remove the ventilator after a couple of hours & keep the patient on t piece.

If the patient is still maintaining the saturation and vitals are well & good then extubate the patient.
