BSA in m2= √ (ht in cm X wt in kg) 3600

23 lb=23/2.2=10.45 kg

=√(81.28 X 10.45)/ 3600

=√849.37/3600

BSA in m2 =0.235 m2

Recommended dose of the acyclovir= 250 mg/m2 every 8 hours.

hence as per patient's BSA required dose= 58.98 mg every 8 hours.

Physician order= 250 mg of acyclovir tid meand 3 times a day, which is more than the recommended dose hence it is not safe.
