You are the risk manager of a hospital. A nurse from the operating room reports that, during a surgery, the head surgeon did not conduct a “time out” to confirm the side and site of the surgery. You question the surgeon, and he denies the incident. There is no injury to the patient.
What is the nature of the risk? Who is at risk? What additional information do you require? What actions can be taken to prevent recurrence of the incident

In such cases. The patient is at risk. The risk could have been the mistaken identity, change in site or side of surgery .
Additional information can be to check the actual site of patient whether followed our correctly. Counter checking with the other staff present regarding the incident.
Plan of action to prevent such type of incident should be dedication of 2 staff to carry out the timeout. As similar to countercheck of timeout once by a nurse and then a surgeon to avoid risk to the patient.
