Mrs Shiela Drew is an 86 year woman under palliative care for end-stage Alzheimer's disease. She is prescribed subcutaneous analgesics and calmative. Mrs Drew has a subcutaneous catheter insitu and a syringe driver connected. Complete observations for the Subcutaneous catheter site and medication administration. Write Progress notes of your findings.

Mrs.Drew 86 year old female patient under palliative care for end stage alzheimer's disease. She is on subcutaneous analgesics. Subcutaneous catheter in place and syringe driver on place and working well.

So as a nurse need to assess the site for any bleeding ,redness,swelling or any leakage as per visual infusion scale.Need to assess how much amount of medication is adminstered. Ask the patient for pain in numerical pain scale 1- 10. Depends on severity of pain titrate the infusin.

If you find any changes in site like bleeding,swelling or redness then stop infusion ,inform primary doctor and remove the catheter or as per advise carry out the orders.

Dont forget to document the progress notes.
