    HPI: The patient is a 42-year-old gentleman who works at the local garden shop part-time. He explained that yesterday he fell from his pickup truck as he was loading gravel for a landscaping project. He lost his footing when attempting to climb from the bed of the truck and fell about 4 feet and landed on a rock, striking his head. He did not lose consciousness, but was quite dizzy.     This headache persisted to the point that the patient grew concerned. Self-administration of ibuprofen provided no significant improvement in pain level. Hence, he sought care here. I met the patient after the emergency room physician had cleared him as stable.

    At the time of admission, the patient's review of systems were negative, except for the noted dizziness, headache. He had no blurred vision or confusion, just substantial pain.

    His past history includes being married with 2 children, work at the garden shop since retirement, and he smoke around 2 packs of cigarettes per day, for the past 10 years. He father died of non-specific heart disease at 52. His older brother has some chronic bony spinal problem, but his twin sister is in good health as far as he knows. No known allergies, and no significant surgical or medical history. This includes no long-terms use or prescription of medication.

    His admission exam revealed tenderness, erythema, and localized edema at the occipital region of the skull. No skin lesions or tears were noted per my examination. CT and plain film imaging showed no subdural bleeding or suggestion of encephalitis. He was cognizant and oriented to 4 spheres, lucid and alert. His main complaint was pain.

    I thought it prudent to maintain him on observation status in the proceeding hours as a precaution against concussive symptoms. It is my belief, after re-examination, that the patient is ready for discharge.

    Currently, his pupils are equal, round, and reactive to light. He professes no ringing in his ears. While the occiput area is still exquisitely tender, his neurological exam is fully normal. He expresses an interest to go home and I concur.

    He knows the signs of neurological changes and I have also included his wife in on that discussion. He will return to the ER here for any such changes. Otherwise, he has been given a low dosage of Vicodin for pain, and is asked to follow-up with our outpatient clinic within 2 weeks or with symptomatic escalation.
