You have been weaning another patient, who is on SIMV- 5, VT 475(machine) FIO2-35%, 5cm
PEEP, spont vt= 360, spont RR=13, PS=10, ABG- 7.37, pCO2=45, pO2= 74. The Dr. Wants your
input if this patient can be extubated or not what will you tell them and how did you reach your
decision? what items did you look at what did you order, what parameters did you measures,
what clinical factors among other led you to your answers. (may receive up to 6 extra pts. Based
on quality of answer).
The patient is on SIMV (Synchronized intermittent mandatory ventilation), the SIMV rate is set on 5. The total respiratory rate is 13 ( the patient's own respiratory rate is 8).
That means the patient has spontaneous breathing, which is a good sign of weaning.
FIO2 is on the ventilator is 35% & PO2 in ABG is 74, which indicates the patient indicates the patient maintaining optimum label of saturation with lower FIO2.
Spontaneous tidal volume is 360, which is again a good sign for weaning.
The partial pressure of Carbon Dioxide is 45 & PH is in the normal range, which indicates there is no acid-base disorder & no carbon dioxide retention.
Conclusion- We can start the weaning process by putting the patient on spontaneous breathing trial(SBT) mode with PS- 10.
After a couple of hours, we can decrease Pressure support and progress in the weaning process.
