S72 for femur fracture, 111 for greater trochanter right displaced , D for subsequent encounter.

A) 4. Diagnostic code for dislocation of left clavicle with displacement 100%, emergency department visit is S43.122 A.

S43 for dislocation of left clavicle 122 for 100% displacement A for initial encounter.

B)2. Diagnostic code for neck strain, cervical spine, subsequent visit, physician's office is: S16.1XXD.

S161XX for strain of cervical spine and D for susequent encounter.

B) 4. Diagnostic code for concussion with minimal loss of consciousness is: S06. 0X1A.

S06.0 for concussion 0X1 for loss of consciousness for specified period of time ( 30 mins or less). A for initial encounter.
