There is another crosscutting initiative called the Health Impact in 5 years (Hi-5). These encompass community wide interventions that often address several conditions at once. How do heart disease and strokes fit into the HI-5?

The Health Impact in 5 Years (HI-5) initiative highlights non-clinical, community-wide approaches that have evidence reporting 1) positive health impacts, 2) results within five years, and 3) cost effectiveness and/or cost savings over the lifetime of the population or earlier.  The aim of the HI-5 initiative is to have a "lasting impact on health outcomes" by focusing on the social determinants of health (rather than only on patient care) and on making it easier to make healthy choices. HI-5 highlights 14 well-known, already proven community-wide interventions . Each intervention has had a positive impact on health, has seen results within five years or less, and has been cost-effective or has cut costs over the lifetime of the population or earlier.

• School-based programs to increase physical activity.
• School-based violence prevention.
• Safe routes to school.
• Motor cycle injury prevention.
• Tobacco control interventions.
• Assess to clean syringes.
• Pricing strategies for alcohol products.
• Multi-Component Worksite Obesity Prevention.

Interventions Addressing the Social Determinants of Health

• Early Childhood Education
• Clean Diesel Bus Fleets.
• Public Transportation System Introduction or Expansion:.
• Home Improvement Loans and Grants.
• Earned Income Tax Credits.
• Water Fluoridation.

While viewing the interventions of HI-5 policy we can see that the first intervention itself is directly related to the prevention of heart disease and stroke in their adulthood. increasing physical activity can prevent obesity and can maintain their body fit. hence the people will stay away from life style diseases.  

Worksite nutrition and physical activity programs are designed to improve health-related behaviors and outcomes among employees.^[1] Employers may offer worksite weight control interventions separately or as part of a comprehensive wellness package that addresses multiple health issues (e.g., smoking cessation, stress management, cholesterol reduction).^[2] Worksite strategies can include one or more approaches to support behavioral change .. hence these plans also help to reduce the stroke and heart disease.

In this way heart disease and strokes fit into the HI-5.
