Needs help for Concept Map with an elderly patient that has depression due to lost of spouse for about 10 months, can no longer do ADLs, having loneliness feeling, feeling very sad, has limited friends around because majority of his friends had passed away, the only child hardly comes around, feeling down for several months and recently having suicide thoughts. He was diagnosed with Mitral Valve Repair and his blood glucose was 270 mg/dl on admission. Needed to be discharged back home.
