Section 563 is dealing with all types of rules and regulations related to medical records. In that 563. 12 is dealing with the hospital record and completion. The provisions of chapter 563 issued under Health Care Facilities Act , Chapter 8.

563.12 : Form and content of record

Every record shall be accurate, legible and complete.  

The retention period for patients and minor patients.

The Health Care Facilities Act also have the written policy regarding the retention of records.

* Medical Records shall be kept on file for a minimum of 7 years after the discharge of the patient.

* If the patient is minor, record shall be kept on file until he become a major person and then for 7 years as the e record keeping of an adult.

Requirements for Medicare specific health record retention

For Medicare purpose, a Physician has to retain the medical record for six years at least from the date of its creation or the date when it last was in effect

Joint Commission Requirement for health records

Healthcare records must be retained for 7 years.
