• To plan for care that is safe (history without knowing allergies, medical conditions, diet, lifestyle, alcohol, smoking, abuse ,etc.,) can endanger the treatment in case of any allergies or any medical conditions
• To provide an informed care
• To convey information to others in the team and the professionals who are involved in care of the patient.
• Here the patients history will enable to identify any risk associated with mental health in relation to the age (who are prone for substance abuse, violence ,etc.,)
• It can give an idea of the behaviour of the teen at this age ( any sex couples activity, peer pressure, etc.,)
