10. Patient admitted for recent memory loss of questionable etiology, rule out cerebrovascular accident and rule out brain tumor. She went to a church meeting and suddenly got up and told everybody that something was terribly wrong and that she could not remember anything, and she seemed quite confused. The patient was oriented; however, she has a problem with her recent memory, and she does not remember leaving the house this morning, driving to church, and going to the meeting or who she was sitting with or anything. The first thing she remembers is being in the emergency room. She seemed to have a retrograde amnesia but seemed to be intact neurologically otherwise. Urinalysis was negative. Glucose fasting was 165, which is somewhat elevated. Electroencephalogram (EEG) was normal. Cerebral flow study and brain scan were normal. CT scan of the brain was normal. The patient's course in the hospital was one of rapid improvement. Blood pressure came down after the administration of intravenous antihypertensive medications. She acted perfectly normal except for the retrograde amnesia, and this persisted. By the day of discharge, she did not remember any more about the day of her admission than she did in the beginning. She was seen in neurological consultation, and it was determined that her problem was one of transient global amnesia that is most commonly not a recurrent thing, and is believed to be due to some sort of a vascular spasm. She is being discharged home, taking Dyazide once daily for her hypertension. FINAL DIAGNOSES: Transient global amnesia. Hypertension.

12. Patient is admitted with acute ST elevation (STEMI) anteroseptal myocardial infarction. This 62-year-old man stopped his Procardia one month ago and had an onset of precardial pressure going up his neck and down the inside of both arms at 10:30 this evening. This lasted for approximately one hour. He was brought to the emergency room where, upon EKG, he was found to be having an acute ST elevation (STEMI) anteroseptal myocardial infarction. Chest x-ray showed chronic obstructive pulmonary disease. Upon admission, creatine kinase (CK) isoenzymes were 89; later that evening total CK was 1680. The following day, CK was 1170. Urinalysis was normal. Electrolytes and complete blood count were normal. Arterial blood gas on 4 liters revealed PO2 of 73, PCO2 of 40, pH of 7.43. On the day of discharge, PO2 on room air was 67. In the intensive care unit, his Procardia was restarted, and lidocaine drip was begun. Lasix was also added to his IV. He had no further angina. His lungs remained clear, and telemetry showed normal sinus rhythm. After 48 hours, the lidocaine was weaned, the patient suffered no arrhythmias, and he was moved to the floor. He was able to walk in his room without angina. He remained out of failure and free of angina or arrhythmia. FINAL DIAGNOSES: Acute ST elevation (STEMI) anteroseptal myocardial infarction. Chronic obstructive airway disease. Arteriosclerotic cardiovascular disease. Status post myocardial infarction in 1977.
