You are the RM of a small, local, long-term acute care/rehabilitation facility in your hometown. Your CEO informs you that referrals from hospitals, physicians offices, worker's compensation companies and nursing homes have sharply decreased over the last 6 months. Enough so that there will be changes made to the budgetary projections for the next fiscal year. It is your job and priority as the RM in charge to create a Risk Management project to improve referral numbers and the facilities reputation in its referral area.

Your plan should include:

Action Plan: how to stop the decrease in referrals, the counterattack, monitoring the competition, in-house clean-up, marketing plan, etc.

Steps of the action plan :-

1. Choose an appropriate goal and clearly define your objective. In this case , the goal and objective is to increase the number of referrals and reputation of the Organization.

2. Use a team to create your action plan . In this case the team is the other co-workers working in the institute . Providing them training and education to improve the quality of care by analysing their strengths and weaknesses etc provided to the patients and making decisions by taking the suggestions from their side will help to create a attractive and efficient healthcare plan and services which would hence help to increase the number of referrals .

3.Choose action steps that are concrete, measurable and attainable. These steps should be clearly defined, not vague ideas. Some of the steps include :-

- provision of quality care

- low cost for the services

- homely and culture friendly services

- home services at door steps

- analysis of threats and opportunities

4. Identify who is responsible for each action step. In this case the actions has to be taken by the healthcare workers working in the Organization including physicians ,nurses etc

5. Provide a clear schedule for completing action steps. Set a particular time for the accomplishment of the task

6. Review and update your action plan as it is implemented. As you track the progress of your plan, make any changes needed as they arise.

7. Communicate with key people about the plan’s progress and effects as it is carried out
