Patient safety and quality improvement act emerged in the year 2005. Its purpose was to establish a voluntary reporting system which improves the data accessible to assess and resolve patient safety and health care quality issues.

The three main goals of patient safety act are:

- encourage

- promote

- share

Patient safety work product according to the act is stated as;

" Any data, reports, records, memoranda or analyses that are assembled or developed by a provider for reporting to a pstudent safety organisation ,

are reported by the pso , are developed by the pso for the patient safety activities or that identify or constitute deliberations or constitute information reported through a patient safety evaluation system".

It is used in order to safeguard the patient's information by the health care providers. The patient's information should not be disclosed or discussed with others as well as patients information should not be written or maintained separately other than his medical file or record. It intends to provide the patient's report to the patient safety organisation directly

Open disclosure is an open discussion of incidents that result in harm to a patient while receiving health care with the patient, their family, careers and other support persons."

The civil monetary penalty assessed against a person who knowingly or recklessly divulges patient safety work product is not more than $ 10,000 for each act constituting such violation.
