Briefly describe one community health problem from your community’s health improvement plan (Healthy babies and Moms - Eliminate or reduce infant mortality by increasing the pregnancy interval to greater than 18 months) . What structure, process, and outcome standards would you use to evaluate a program addressing this problem?

The main reason of infant mortality is low birthweight baby by which their growth not increases as pre natal care is not proper.in some issues it found that environmental condition also effects baby and mother health.some deaths are due to birth defects which are due to deficiency of nutrition in mothers body. If proper psychological support is given to mother then infant mortality can be reduce.some mother consumes alcohol and smokes during pregnancy which leads to death.

Nutritional pattern of mother can also effects on breastfeed if community worker or through this program we can educates the mother about their diet,habbits,practices of breastfeeding,care of neonates and education on immunization then only many infant can be survive.Proper contraception methods to control births can also help into reducing it
