* Calculation of dose(mg) required per week.

         Ordered dose = 25mg/m2/week.

         BSA of the patient = 0.62 m2.

   Dose required per week for this patient = 25 * 0.62

                                                                   = 15.5

                               Round to the nearest tenth

                                                                  = 16 mg/week.
