Ordered methotrexate= 40 mg PO twice a week (PO means orally)

Recommended dose= 20-30 mg/m2 twice a weak

The recommended dose as per the BSA of patient = 20 mg X 2.12 to 30 X 2.12 mg

An effective safe recommended dosing in this patient   = 42.4 mg to 63.6 mg twice a week

Since the ordered methotrexate dose i.e. 40  mg PO twice a week and it is within the individualized safe dose range of patient (42.4 mg to 63.6 mg twice a week), Hence is safe.

Means option C is correct.
