Patient: Blaize Masters

IMPRESSION: This is a 49-year-old male who has significant multivessel coronary artery disease. He has atypical anginal symptoms, suspected to be secondary to his type 1 diabetes mellitus. Still it is believed that he is at risk for ischemia. To reduce this risk, surgical myocardial revascularization is recommended.

The patient is given complete details about the procedure, the risks, and the benefits. We also discuss alternative treatments that may be viable. The patient signs the informed consent and the surgery is scheduled.

PLAN: Proceed with coronary artery bypass graft operation utilizing the left internal mammary artery as conduit to the left anterior descending. The remaining conduit will come from the greater saphenous veins.

Be sure to list the codes, one code per box, in the correct order, from top to bottom. Capitalization, punctuation, and spacing can impact whether or not your answer is correct. Follow coding best practices.
