1, Nursing process for Cholecystitis:
cholecystitis is the inflammation of the gall bladder..when there is blockage between gall bladder and small intestine,bile accumulate inside the gall bladder and cause inflammation..surgical removal of the gall bladder is needed..
patient have a symptoms of nausea,loss of appetite,abdomen pain and there will be signs fever,vomiting,jaundice,abnormal lab values,clay-colored stools..
Nursing process included in cholecystitis
-Assess the vital signs,patient may have fever,tachycardia due to inflammation...
- listen abdomen sounds for belching,look for distention,feel for tenderness.
- make IV access ready for any surgical procedures..IV solution administration and medication..
-We can control the symptoms pain with medication..make patient comfortable in bed with semi-fowler's position
-Provide adequate bed rest
-administer antibiotics,Antiemetics,Analgesics to manage the symptoms as per doctors advice..
-Do routine blood investigation of liver function test,urinalysis, ultrasound,CT, MRI, as per physician order.
-Prepare patient for diagnostic procedure for endoscopic retrograde cholangio pancreatography(ERCP) and Hepatobiliary Iminodiacetic Acid scan(HIA) for investigation..
2, Discharge planning:
-Advice patient about weight control,encourage diet and exercise.
- Advice to avoid foods like pork,gravies,butter,fried foods etc..
- Advice to avoid alcohol,coffee,tea etc..
-advice to avoid gas producing foods like cabbage,beans,carbonated beverages..
-advice to come for routine followup
-If there is any pain or any side effects increased should contact the doctor immediately
- Add fiber rich diet
3, encourage the patient to eat low fat diet to reduce the weight..low fat diet in cholecystitis helps to reduce the weight and prevent fat deposition in the gall bladder..
advice patient to do regular exercise and eat three balance meal every day..In cholecystitis it helps for weight reduction and reduce pain.
4, NANDA nursing diagnosis:
Acute pain:
- Check pain scale the level of pain for the patient
-Administer pain medication and Anticholinergic as per doctor advice..
- Promote adequate bed rest
-Provide relaxation technique and make pleasant environment..
Fluid volume Deficient:
- Monitor intake and output record
- Assess patient skin and mucous membrane for any dehydration
- Administer Intravenous solution infusion as per doctor advice..
Imbalanced nutrition:
- Calculate patient nutrition requirement.
-check patient weight daily
- Provide pleasant atmosphere at meal time
- Get dietition opinion for nutrition requirement
-check patient abdomen distension and pain, and bowel sound
- Increase activity as tolerated.
Knowledge Deficit:
- Explain patient about their condition,test procedure and followup..
-Keep proper communication with patient and family to avoid fear..
- Advice to avoid high fat,gas producers etc..
-Explain about signs and symptoms..
-Explain about comfortable position (semi-fowlers) after meal.
- advice to avoid aspirin and it containing products..
