      Dose ordered for the patient = 50 mg / kg IVPB q 8 h.

    Weight of the patient = 60 kg.

      So

          Dose required for this patient with 60 kg body weight = 50 * 60

                                                                                               = 3000 mg (q8h)

           Recommended dosage range = 250mg to 2 gm every 8 -12 hours.

                     Convert gm value in the dosage range to mg for identify the safe range in mg.

                           So

                                 Recommended dose range = 250 mg to 2000 mg every 8 - 12 hr.

           * Patient dose (3000mg) is not within the safe range
