Ted is a 48 у.о. mi ale admitted to the surgical floor for an exacerbation of Crohn's disease (severe inflammation of the small bowel usually but can affect the GI tract anywhere). He is currently NPO and will be starting PN before being discharged in a few days. He will be on PN for 6-8 weeks due to the presence of a high output fistula. A PN consult has been ordered. Calculation used to estimate caloric needs is based on a standard number of calories per kilogram (kcals/kg) of body weight per day. The typical standards are 25 to 30 kcals/kg/day for normal, healthy individuals; 30 to 35 kcal/kg/day for patients with moderate illness, injury, or malnutrition; and 35 to 40 kcals/kg/day for patients with critical illness or injury.

# In this case the patient is critically ill ,so the calories needed by this patient is 35 to 40 Kcals/kg/day

Weight of the patient is 170lbs = 170×0.45kg = 76.5 kg

Calories = 35× 76.5 to 40×76.5 /day

= 2677.5 to 3060 Kcals

# Protein requirement according to RDA for this patient is from 0.8 gm -1.8gm /kg ,

= 0.8 × 76.5 kg - 1.8 ×76.5

= 61.2 to 137.7 gm for proper healing and recovery /day

# Fluid requirement for more than 20years of age :

1500ml+ 20/kg

= 1500ml+ 20× 76.5 kg

= 3030ml/day
