She comes into your ER with complaints of pain at infusion site and thirst. Her assessment shows dry flushed skin, HR 100, RR 26, B/P 100/60, T101F, Blood sugar 450. She also has large red swelling at site of pump infusion. After reading the above findings, how would you interpret the information? What are your priority interventions? Teresa was diagnosed with DKA and was placed on an insulin drip. As her blood sugar decreases, what are the chief concerns the nurse needs to monitor for? Teresa recovers and says that she had not felt well for 2 days before she was admitted. What does the nurse need to teach Teresa about caring for herself during an illness or infection? After reading the above findings, how would you interpret the information?

The assessment fining shows that the child had large red swelling in the infusion site, temperature is 101-degree F and glucose level of 450. These can clearly indicative of an infection in the infusion site and the insulin which were not able pump to the blood adequately

What are your priority interventions?

The priority of nursing intervention should be focusing on the child should be free from infection as evidenced by the lowering of the temperature. Adequate care should be taken to the swelled area and clean it properly and correction of the blood glucose level.

what are the chief concerns the nurse needs to monitor for?

The nurse must be concerned regarding the control of the blood glucose level of the child, caring of the infusion site to prevent the further infections

What does the nurse need to teach Teresa about caring for herself during an illness or infection?

The things should be noted that if she had any problem with the infusion site or feeling of uneasiness should be reported to the parents or teachers, and the blood glucose should be monitored at that time.

What does the nurse teach Teresa about monitoring for hypoglycemia

If she had having feeling tired, not able to do anything, having dizziness or syncope she may have the signs of hypoglycaemia she should take some sweet kind of things and also to immediately check the glucose level.
