A 98 years female is admitted to the unit with complications of UTI and C. Difficile. The patient states, “I peeing is so painful and smelly, and I can even see the blood.” There were also reports of her having to go to toilets so frequently and that made her very tired. She was in the treatment of C. Difficile too. Her vitals are normal.

SOPA (Subjective,Objective,plan,Assessment) note.

* S - Subjective.

      The patient states " I peeing is so painful and smelly and I can

      even see the blood.

      Reports of her having to go to toilets so frequently and that made her

      very tired.

* O - Objective.

      Vital signs are normal.

* P - Planning.

      Treatment for C.Difficile and UTI(Urinary tract infection).

* A - Assessment.

       Patient has complications of urinary tract infection and C.Difficile

      and she was in the treatment of C.Difficile.
