1. I would report about the bowel movement of patient as well as the complain of tiredness as said by the patient after walking for a distance of 20 feet after breakfast, as his age is 75 years it is important to record the minor changes of the body to prevent further complications.

2. I would inform to the concerned physician of the nursing center to make the needed assessment and do the needful treatment plan of action for the patient.
