Physical examination reveals an overweight woman who is short of breath from the walk to the examination room. Her fingers are puffy with tight rings that she is unable to remove. The woman reports she has a history of cigarette smoking, primary hypertension, high cholesterol, and type 2 diabetes. Based on risk factors and your initial assessment, you swiftly make arrangements for this woman to be transported to the nearby emergency department.

57 year old female lady presented to clinic with headache which is not subsided with acteaminophen and ibuprofen.She is obese who has shortness of breathing while walking.On examinations her fingers are puffy due to obesity and tight rings that she unable to remove. She has history of smoking, primary hypertension, high cholestrol and diabetes.

My initial assessment will be initial vital signs to check blood pressure and spo2 levels as she has headache and shortness of breathing.

check random blood sugars as she has diabetes.

so should be transported to higher center where they can do ct brain as she has headache and HTN so we can check any bleeding in brain and review with medicine doctor for her obesity and high cholestrol levels, she is known smoker can do one chest xray etc.
