The five steps of nursing processes are,

1. Assessment

During the assessment stage, the nurse gathers information necessary for identifying the client. The data collected includes both subjective and objective data. The collected data is analyzed systematically and then used to state the “nursing diagnosis.” Based on the diagnosis, appropriate treatment is planned and implemented. During the evaluation stage, the patient’s outcomes are compared with the expected outcomes or planning goals.

The evaluation includes,
