Mr Bob Morley is hospitalised for management of symptomatic anaemia from chronic gastric ulcers.

He has a history of congestive heart failure and hypertension.

Bob is Short Of Breath On Exertion, lethargic, feels dizzy at times and is .

Tachycardic.

Bob's Haemoglobin is 68g/L

1 unit of Packed Red Cells has been ordered.

Your Task: Check the medication chart and blood transfusion order. Administer pre-medications and blood products as prescribed. Explain why the client may be having the pre-medication.

Ans) New case admitted.

- Patient is conscious & oriented.

- He has anaemia with HB = 68g/dl & K/C/O CHF and hypertension.

- Vital signs checked and recorded.

- Admission informed to Assistant Dr.

- Diet informed to Dietician.

- IV line assessed.

- I pint PCV to be arranged & get it.

- Take blood transfusion consent & check for all necessary detail.

- Pre medication- Such as avil, lasix , hydrocortisone may be advised to administering.

- Start the PCV & check vital sign.

- Adjust the drip rate accordingly.

- PCV to be administered in 4-5 hours.

- Keep strictly blood transfusion record.

- Notify any deviation.

- Return the empty blood bag.

- Sign the PCV order.
