A young lady, age 15, is brought to the school nurse after fainting during gym class. She is grossly underweight, wears baggy clothes, and has dry skin. She complains of feeling cold despite wearing two sweaters. As the nurse examines her perfunctorily, she notices that young lady skin seems slightly yellow, and a gentle scrape of her skin shows signs of dehydration. 1. Based on the findings, what is the likely diagnosis? 2. What are contributing factors? 3. What would be an appropriate treatment plan? (Include possible medications, interventions, etc.). 4. List one short term and one long term goal for this client.

Ans1: A young lady who is 15 year old , the nurse diagnosed that a young lady is grossly under weight and has dry skin and gently scrape of their skin which indicate that a young lady is suffering from dehydration but there is indicate that a young lady should wear two sweaters which indicate that a lady should suffering from severe coldness ,these can occur when a body having less vitamin B12, that lead to B12 deficiency anaemia or a low red blood cell count result in to chronic coldness. Here there is indication that a lady skin seem slightly be yellow , these indicate that there is high level of bilirubin in the body which make the body yellow, . It indicate that a young lady may be suffered from jaundice( it is a condition in which skin sclera and mucus membranes turn yellow.

Finally a school nursed should diagnosed that a lady may be suffered from dehydration, severe cold and jaundice.

Ans(2):Following are the contributing factor for diagnosed dehydration,severe cold ,jaundice.

(a) Dehydration:Dehydration is usually caused by an inadequate intake of fluids.other contributing factors include climate,physical activity and diet.Dehydration is also caused by illness that may lead to fluid loss such as persistent diarrhoea and vomiting.

(b) Severe cold : These may caused due to a many type of viruses.It may also caused due to deficiency of vitamin like vitamin B12.Other contributing factor are climate change ,body weakness etc.

(C) Jaundice: it is caused by high level of bilirubin ,yellow orange bile pigment.Bile is a fluid secreted by the liver.Bilirubin is formed from breakdown of red blood cells.other contributing factor are dehydration,illness,body weakness etc.

Ans(3): following are the appropriate treatment plan,

(a) Dehydration : A young lady may be suffered from Dehydration so that it is essential that a body should be fulfilled with electrolyte, for that ORS ( oral rehydration solution ) is given to young lady after consciousness for temporary treatment .For secondary treatment a school nurse should advised the lady that he should dink energy drink regularly,drink more amount of water, decreased eating food that increased urine flow ,should be protect from sun for 10 to 20 day so that the body should be less dehydrated.

(b) severe cold: A young lady may suffered from severed cold so for that nurse advised to eat a food that contain lot of vitamin B12 like egg, fish liver oil etc which increased the vitB12 level in the body which decreased the chances of severe coldness, also adviced to increased the weight according to their age by eating healthier food and healthier exercise.

(C) jaundice: A young lady may possess a slightly yellow skin which may be indicate that she is suffer from jaundice for that nurse should adviced the following point.

(1) Drink a plenty of water during whole day nearly eight glasses.

(2) opt for fruit like papaya and mango which are rich in digestive enzymes.

(3) Eat high finer foods such as almonds ,berries etc.

(4) Eat at least 2 1/2 cup of veggies and 2 cups of fruit per day.

(5) Drink at least one cup of milk daily

Ans(4) one short term goal is too full filled the body requirement by drinking ORS,by drinking energy drink , drinking lot of water. etc

A long term goal for a client is that .a client should over come the under weight by doing regular exercise in gym ,eating health and fibrous food and tried to maintain their body as per requirement.
