You are the Production Department manager at textiles manufacture, and last month there were complaints from customers that the quality of the products shipped to them was lacking. Some stitching on T-shirts was weak and the sizes of socks did not meet specifications.

The Operations Manager asks you to conduct a root cause analysis and report back to him your findings and he wants you to prepare a memo detailing how to refocus the organization on quality.

Create a plan for how to conduct a root cause analysis to investigate this problem. Also, prepare a memo to the Operations Manager detailing memo how could be utilized in the organization to improve quality.

Root cause analysis for the poor quality of product our textile company manufacture will be done by following the below mentioned approach

• Brainstorming of what we have done and how such low quality product shipped to our valuable customer. It will be done in the department where there will be quality, purchase and production department’s representative will join.
• The 5 whys, where 5 why will be asked till a conclusion will not be reached.
• Fishbone diagram approach where various causes will be grouped into category with arrows in the image indicating how the causes flow toward the nonconformity. It will be extensively used to draw conclusion regarding what wrong we have done in maintaining the quality in our product.

We have made some careful study about the complaint our organization has received from some of our valuable customers regarding shipment of substandard product to them which is against our ethics and code of business. As the head of production department, I have had consultation with all the related departmental head and come out with solution on improving quality.

We have planned to change some of our qualitative measure by analyzing root cause analysis of this problem by adopting various techniques and tools which are available to do so. We will have a quality circle which will oversees the quality related issues. It will report jointly to me and to quality head. We will also encourage our employees to make kaizen which can increase our quality much more and workers will also be trained more about quality.
